I, being of sound mind and body, hereby release 

any and all persons representing The Parlor Suites from all responsibility related to my request to have tattoo work done. I accept any and all responsibility myself for any consequences that might stem from my request to have tattoo related work done by the artist within The Parlor Suites.

I agree not to sue the artist or The Parlor Suites in connection with any and all damages, claims, demands, rights, and causes of action, of whatever kind of nature, based upon injuries or property damage to myself or any persons arising from my decision to have tattoo related work done at this time, whether or not caused by any negligence of the artist.

I agree for myself, my heirs, assigns, and legal representatives to hold the artist and The Parlor Suites harmless from all damages, actions, causes of action, claim judgments, or costs and expenses which might arise from my decision to have any tattoo related work done by the artist within The Parlor Suites.

I ACKNOWLEDGE THE FOLLOWING:

  • I am AT LEAST 18 years old.                                                   

  • I am NOT under the influence of drugs or alcohol.

  • I am NOT pregnant or nursing. 

  • I DO NOT have a heart condition or epilepsy and DO NOT take medications that thin the blood.

  • I DO NOT suffer from hemophilia or skin conditions such as, but not limited to, acne, scarring (Keloid), eczema, psoriasis, freckles, moles, or sunburn in the area to be tattooed that may interfere with the tattoo. If I have any type of infection or rash anywhere on my body I will advise the artist. 

  • I HAVE advised the artist of any allergies to soaps, ointments, metals, or latex and acknowledge that it is not the responsibility of the tattoo artist to determine whether or not I may suffer a reaction to materials used in the process of tattooing.

  • I realize that variations in color and design may exist between any tattoo as selected by me and as ultimately applied to my body. I understand that if my skin color is dark, the colors will not appear as bright as they do on light skin.

  • I understand that if I have any skin treatments, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to my tattoo.

  • I acknowledge that a tattoo is a permanent change to my appearance. To my knowledge, I DO NOT have any physical, mental, medical impairment, or disability which might affect my well being as a direct result of my decision to have any tattoo related work done by the artist.

If I am unable to confirm any of the above mentioned points, I have informed my artist and I will assume all responsibility thereafter.

I will receive aftercare instructions from the artist and I agree to follow all instructions concerning the care of my new tattoo. I have been given the opportunity to ask any and all questions that I may have at this time and further acknowledge that I have received the email for the artist and may contact them at any time with any questions. I further understand that infections are always a possibility with any tattoo.

All needles and tubes are used once and then properly disposed of. I am always welcome and encouraged to watch the needle(s) used on me be placed into a biohazard waste container for future disposal.
